About the Role

We are seeking a results-oriented, team-focused and mission-driven professional to join our team as the Director, Practice Transformation. The practice transformation team is responsible for partnering with providers and clinic staff and working across the Hopscotch team to define the strategic and operational roadmap for clinics we acquire, implement changes in line with the roadmap, and managing metrics to achieve desired clinical, operational and financial outcomes.

As a master change agent, the Director of Practice Transformation will need to be comfortable working at all levels, in the business and within the organization, and empowering a team to observe current practices, develop a plan for change focused on the major priorities and execute against the plan, supporting the providers and staff at every step. Communication and collaboration, with providers, across the clinic and with Hopscotch senior team will be critical to enable success.

Our ideal candidate will bring skill, tenacity and passion to play an instrumental role in strengthening the Hopscotch business strategy, product roadmap and care model. The location of this role is flexible. Travel to the Western North Carolina market will be important.

What You'll Do

The Director, Practice Transformation will play a critical role on the senior team at Hopscotch and will work closely with the CEO and leadership team, especially in the first 12-18 months in the role.

This position will support our practices in the Western North Carolina region.

Specific responsibilities for this role will include, but are not limited to:

  • Lead all things Practice Transformation (with accountability for results):
    • Develop and maintain strong relationships with providers, clinic staff and across Hopscotch team to enable trust and results in the context of all Practice Transformation efforts. Prioritize relationships and trust at every step.
    • Develop and leverage an assessment tool for practice acquisition candidates, clinical, operational and financial within the practices we are acquiring.
    • Develop a practice transformation strategy and roadmap for each acquired practice, based on the current state, assessment around ability to change, and prioritization of the most important changes to drive clinical, operational and financial results plus patient and provider satisfaction
    • Execute against the practice transformation strategy, monitoring performance against key metrics and working closely across the team to address opportunities and risks as they emerge
  • Support other critical priorities for Hopscotch, including but not limited to:
    • Codify lessons learned by building a practice transformation playbook to make the process more efficient and effective with each successive acquisition
    • Develop patient engagement strategies for acquired and de novo clinics
    • Serve as an ambassador in the market for Hopscotch, developing relationships, articulating the vision and building our ecosystem of partners and talent
About You

You would be a great fit for this position if you have 10-15 years of experience working in clinical operations, business/operational excellence, and provider services, ideally with progressive models and you are:

  • Mission-driven and seek to serve patients and partner with providers to make lasting change in healthcare
  • Experienced in partnering with clinicians to drive change management and implement innovative care models; able to take a relationship-first approach to driving collaboration and results
  • Strong with program management skills and able to drive execution against a roadmap
  • Results-driven and can focus time/resources against the most important priorities
  • Comfortable with ambiguity and can bring a structured, proactive approach to get things done and execute
  • Strategic and able to develop robust insights from data and analysis, with an ability to translate findings into action
  • Articulate and succinct with communication about complex concepts, verbally and in writing, and use synthesized communications to drive results
